Previously owned, donated hot tubs through our Hot Tubs for Heroes program. Please indicate the hot tub you are applying for in the field below.
There will be many factors considered when selecting the final recipient. However, at a minimum, the following is required:

All recipients must own their own home. They must be current on their mortgage and submit a copy of their current statement with this application.
He / She must have a physical and/or psychological impairment that would benefit from hydro-therapy.
Must hold a status of Active Duty OR Veteran.
Must submit copy of DD214 (Member 4 Copy).
Must submit a copy of VA Rating Letter that provides nature of disability(ies).

By submitting this application/nomination, I affirm that the facts set forth in it are true and complete to the best of my knowledge. I understand that all submissions will be reviewed and that a “short list” of candidates will be created. As of that point, a site visit will be required prior to final recipient being named. Any, statements, omissions, or other misrepresentations made on this application may result in disqualification. If selected as a recipient by WFAW, and the veteran moves within a 3 year period, (without extenuating circumstances accepted by WFAW) the veteran will be responsible for reimbursing WFAW up to the value of their donation.
